Currently, for Niagara College Staff and faculty the daily health screening form can be accessed using the Employee Self-Serve page. However, during scheduled maintenance of the HR system the health screening form will be unavailable. During this time there are two alternative options that can be used to complete the health screening. Please note, that there will not be an alternative option for OnCampus requests. This means all requests to be on-campus need to be submitted and approved prior to the HR system coming down.
Option 1 – Niagara College Mobile App (available October 2nd)
Niagara College has a mobile application that has previously been targeted exclusively for students. Students have already had the option of completing their health screen using this method; it has now been update to allow staff and faculty to use as well.
The NC Mobile app supports both Android and iOS and can be installed by visiting the respective platforms app store.
To install the Android version, search Google Play for “Niagara College Mobile”, or use this link to directly access it: https://play.google.com/store/apps/details?id=ca.niagaracollege.ncmobile
Once installed, log in using your Niagara College username and password. Once logged in, open the side menu on the left by either tapping the menu button on the top left or swiping out from the left side of the screen. Select “Daily Health Screen” from the menu (shown in figure 1), complete the form (shown in figure 2), and tap the submit button at the bottom. A successful submission will display a green checkmark to indicate an approved status, as show in in figure 3.
For iOS, open the App Store and search for “Niagara College Mobile”, and install. Once installed, open the app and login using your Niagara College username and password. Once logged in, on the bottom of the screen select “Health Screen”, and tap the “Complete Health Screening” button as shown in figure 1. In the subsequent screen shown in figure 2, complete the form and tap the “Submit” button at the bottom of the form. Once completed, the approval status will be shown. If approved it will display the screen shown in in figure 3.
Option 2 – Web Form (available October 26th)
Note: You need to know your Niagara College Employee ID in order to use this method.
If unable to use a mobile device, a web form version of the health screen is available at: https://ess.niagaracollege.ca/OnCampusCheckpoint/Screening
Once loaded the first page will ask for the user’s Employee ID as shown in figure 1. Enter in your Employee Id and click “Next”.
If you entered in a valid Employee Id you’ll get the form shown in figure 2. Complete this form and click submit. The approval status will be shown immediately on the page.
